docs(mcp): document tool confirmation guard configuration

Add documentation for the MCP bridge's tool confirmation guard feature, including environment variables LLM_MCP_SKIP_CONFIRM and LLM_MCP_NEED_CONFIRM that control which tools require user confirmation before execution. Adding this documentation specifically to the mcp bridge README, where it will be more directly relatable to mcp users instead of in the main readme as in PR #205
