Unable to Add Instance to Instance blocks in account settings

Whenever I try to add an instance to block, after hitting OK the text entry box stays in the screen, and nothing happens despite the "OK" button being greyed out after pressing it. You can tap outside the text box and it disappears so the app is not totally unresponsive, but the instance does not get added.
