Add debug detection and assertion for stale read transactions in NostrDB

As a Damus developer who is trying to improve the quality of the app, I would like to automatically detect stale read transactions in NostrDB on debug builds, so that I can identify txn handling issues and avoid issues such as runaway storage use. **Is your feature request related to a problem?
