A105: MAX_CONCURRENT_STREAMS Connection Scaling

Currently in throes of trying to tune connection pool sizes and stumbled across https://github.com/grpc/proposal/blob/master/A105-max_concurrent_streams-connection-scaling.md. Would love to see this come to life in the Golang lib.
