Allow data packs to add custom entity equipment tables

The `ENTITY_EQUIPMENT` table is currently hardcoded and won't accept custom entries. I use this feature in my pack to scale artifact equipment chance with difficulty, and I'd like to be able to add tables for other modded mobs. Related to #491.
